    Self Defense Instruments

Not specifically referencing that but at 10 feet, I don't care how good you are, most in shape people could have stabbed you, probably multiple times, prior to you firing a shot or getting it drawn. Let alone putting a gun less than 3 ft away from a patient in a metal box where your focus should...

    Self Defense Instruments

As an expert I am sure you are familiar with the studies and experiments showing how effective guns are with an attacker of any kind within 10-15 feet of you...for those who don't know, they are flat out ineffective. I don't care who you are, putting a gun in the back of an ambulance with...

    77 y/o female respiratory distress

Sorry I was referring to the patient I had and not his scenario, My patient did not require CPAP and was pre flash pulmonary edema enough that just BP control without opening the lungs fixed everything. In his patient with this scenario I completely agree that nitro would probably help, but I...

Length of Postical Period on Children with Febrile Seizures

My concern was the kid came around while we started the IV, but 10 minutes later I could pinch the backside of the thigh, tickle, rub, ..while holding one eye lid open and there was zero response, no pupil response, no twitching, nothing at all...it struck me as very strange...I was getting...

Length of Postical Period on Children with Febrile Seizures

Had a call a couple days ago with a 19 month old, called out for unresponsive not breathing. Upon arrival child is breathing but unresponsive to painful stimuli, feels very warm to the touch, family describes seizure like activity prior to going unresponsive. Just curious about personal...

Leaving on the lights while on scene?

Get cut off as we are pulling in the driveway. Usually get cut off pulling into the neighborhood. Usually don't get cut on in the first place because we have discretion as to whether or not to run lights and sirens, if we don't think it will save time we don't have to run them even on priority 1...

    77 y/o female respiratory distress

Much better than slapping on an inch of nitro in the name of chest pain...at least IMO. Nice that you see the effects immediately and it doesn't continue to tank pressures, easy to give repeat doses to hit target pressures.

    77 y/o female respiratory distress

Doc ordered me to give Labetalol to the last patient I had with a pre flash pulmonary edema presentation, BP was 240's/130's when I started medicating and I dropped it down to about 150/90 with improved lung sounds without having to CPAP the patient or use any oxygen...

Rma for minor via cell phone (nj)

Absolutely not. I would advise the person that if I check them out they then become my patient and if they are my patient things might get complicated. If they would like to refuse being checked out all together and they are never under my medical care then there is no problem...Its totally up...

    Nitro or not?

Short answer is no, Patient will get a hefty dose of lasix (being that large) in the ER and will hangout and likely get admitted due to lab work that will no doubt be all jacked up. The real question is whether this patient gets diagnosed with CHF or with pulmonary edema due to acute flair up...

No screwing around after the second round of benzo's, if flights not available and we are an hour from a hospital the kid is getting RSI'd now.

Bed Delays.... The foreign concept, only not.

Sadly all of our local hospital utilize hall beds on a daily basis. One of our large level 1 trauma centers has monitors mounted between rooms on the outside specifically for this purpose. I have placed CPAP patients in hall beds before with the patient being continued on CPAP by one of the ER...

Bed Delays.... The foreign concept, only not.

Never had an issue with a bed delay, all of our area hospitals (including large level 1 trauma centers) will put any "non emergent" patient back in the waiting room and have you give report to the nurse at triage. Also every facility I have ever been to utilizes "hallway stretchers". I will have...

Rma for minor via cell phone (nj)

At my current service I have no issue at all taking a refusal from 16 and 17 year olds who appear mentally competent and alert and oriented. Also if you are concerned about minor refusals you can make it very simple. " Ma'am, Before I assess or treat you at all I want you to know that...

    Pre-Hospital post intubation sedation

For a lot of agencies it gets all the way back to the most basic debate of all, and that is what our "job" is. Fact is prehospital pain medication post intubation does not reduce mortality or morbidity in any condition period. If you go back to the mentality that some people have, that all...
