Let me ask you about uniforms...

Class B s/s shirts, white for medics, light blue for basics, all with patches, collar brass, and name plates. Certain members of the organization, from the top down, elect to adorn their shirts with enough apurtenances so as to be confused with a North Korean general. Pants are required to be...

    Thoughts on High Flow O2

This was a tough one for me. I was raised in the school of thought that oxygen had two settings, on and off. If they needed oxygen, blast em. Nasal cannulas were worthless in the acute setting. For years I held these beliefs as fact. Then, as if a switch flipped, I was awakened to the idea that...

    Lights and Sirens Use

Those are the 80% we don't need, the people holding us back from being a profession. If I could get it in writing that they would indeed quit, I'd rip the lights and whistles out myself.

What is the best multitool knife to get?

Weigh them all and buy the lightest, cheapest model you can find. You aren’t going to use it much at all, so its better if it isnt heavy because thats just adding insult to injury.

    Which type of CPR

I have never, not one of my innumerable participations, seen a textbook code in the field. Never. Regardless of what the plan was when you started, its going to end up being a hodgepodge of ratios and procedures drawn from the various iterations of cardiac arrest management that the participants...

    Am I being impatient?

Hang on. All stop. Wait just one gosh darn minute. You said you applied for 7 job, but six of them are with the same company. So, in reality, you applied for two jobs, one of them six times. One of them turned you down and the other has you in the pipeline. Calm the heck down. Yes, you are...

    Preventing Fraud-"Medical Necessity"

When I did transfer work, at least once a day I would tranport a patient with at best a questionable med nec and at worst a total fabrication. Cardiad monitor for their baseline, well controlled AFib. Tko saline only. Airway precautions for the patients that had received narcotics(hours ago)...

Voting on Question 1 (Massachusetts Ballot Measure)

I believe this refers to Massachusetts. My vote is NO on 1. Clearly and inarguably this issue needs to be addressed, but this proposed action isnt the answer and will be almost immediately deleterious to the state, its healthcare system, its healthcare workers, and the people seeking its services.

NTG and Morphine I’m Inferior AMIs

How’s this for a wild idea. Shoot the right sided cardiogram and see if there’s RV involvement. If there is not, we’re arguing about nothing and if there is, chances are excellent their pressure isnt going to support NTG anyway.

Ambulance Crew Configuration: Are Two Paramedics Better Than One?

99 times out of 100, one paramedic is sufficient. The trouble is, when and where that other call is going to happen is unable to be predicted and thats the one where you absolutely need that second medic and there isnt time for them to come from anywhere but the passenger seat. Is P/P staffing...

    Pain Score and Analgesia

Have some Motrin and an ice pack. I have about the lowest threshold for administering narcotic analgesia possible, but if you're clearly faking or exaggerating, you may place your lips on my buttocks.

    Preventing Fraud-"Medical Necessity"

There exists only one possible solution to this problem, and that is to make individual providers responsible for the fraud. If the doctor, the nurse, and the EMTs were each individually assessed a fine for having lied or been complicit in the fraud, this problem would end overnight. Of course...
