I said normooxemia + bicarbonate. Ofcourse NaBic without adequate ventilation would worse condition. Patiend must be intubated and mechanically ventilated.
:)
In SVT with BP 90 I have used Adenosine.
Tachyarhytmias wit BP lower then 90 - shock condition I would try to do sedation and sync. cardioversion.
Stable AF we treat with Amiodarone, Propafenon, or frequency control by Metoprolol or Digoxine.
Tachycardic AF with cardial insuff. and...
Sorry for my bad english (I understood that episode of hypoventilation appeared there).
So high CO2 (in hyperventilation background) shows that it were metabolic problem.
Intubation end mechanical ventilation -- probably you would ventilate in "normal" frequency 10-12 breaths/min. -- I...
It is great. Do you must work strongly by algoritms or it is allowed to think?
For example patient with regular tachicardia 200 b/min, BP 90, chest pain - do you treat by algoritm or you can decide by yourself --> Amiodarone infusion/sedation and cardioversion?
Sat 93% is not so dramatic, it can by psychical probl.
Thanks hyperventilation his PaCO2 might became low - maybe this was a cause.
As I understand there wasnt episode of apnoe? Maybe unvisible convulsions?
I would intbate him if Sat would fall <90%
I have intubated with a two tubes. It was resuscitation and my first intubation. --> stressful situation and when in laryngoscopy I saw the 1st hole - I put tube (oesophagus:huh:)
I didnt removed it, I put 2nd tube in right hole.
Problem can appear. Intubated oesophagus can compress larynx...
Some times Polish paramedics can call to EM phisician for consult about unstable patient.
One paramedic from Katowice said me that when phician by a phone allows to intubate live patient, they(paramedics alone) intubate using anestetics.
But usually is called Specialized team for help.
Hi!
As I know no paramedics in Russia, there are phisicians, feldsher's, nurses and just drivers in Rus ambulance.
Phisician team has nurse and driver
and feldsher team - feldsher + driver (+maybe second nurse/feldsher.)
Feldsher (chck Wikipedia) - they know medicine, they can...
45.
It is most likely supraventricular rhytm + LBBBB. Maybe hyperkalemia, maybe MI.
If he has chest pain/stenocardia, you should treat him like MI.
Of sublinqual Nitro doesnt stop pain:
-Nitroglycerin IV
-Morphine
-Ac.sal. acid
-Metoprolol or Amiodarone
