Speaking of diagnosis...What have you diagnosed recently?

Last night alone, I diagnosed:

-a DVT in a lady who was 1 week s/p total knee replacement with a positive Homan's sign and redness and streaking up her calf.

-TIAs in a lady with more of the same last week and a full workup then

-really poorly controlled diabetes in a lady with prolonged hypoglycemia (like 4 hours of BGL less than 40 according to her implanted monitor) who took more than half an hour to wake up after she got D50. I had my doubts about whether she would wake up at all.

What have you diagnosed lately?

Spoke to a physician recently who diagnosed paralytic shellfish poisoning. Now that was an interesting differential diagnosis to hear.

The last standardized patient exam had a spontaneous pnemothorax and a DVT. 5 more months... 5 more and this [poop] gets real.
