LA County Local Scope of Practice 2011

I'm about to mail my state license application and one of the requirements is a LA County Local Scope of Practice 2011 certificate. Since I took my EMT class at UCLA, the scope of Practice is already on the course completion certificate so should I just mail as it is or should I do the scope of practice and print out a separate certificate for it?
