ca Ambulance certificate

Hey guys,
I'm a soon to be certified EMT looking for info on acquiring a CA Ambulance certificate from the dmv. Mostly just confused about what form I need to bring to the physical. Most posts only mention the dl-51 but like 3 other forms have been mentioned. I would really appreciate it if someone could help clears this up, really cant find any direct info about it. Any othe info on the process would be awesome too!

Medical examiners card, 2 forms of id, emt card your CA one not the nremt one, pay the $5 take the test.
