Confused with my cardiology - why do we reverse lead IV on a 12-lead? I must have blanked out during lecture because this part completely escaped me and I can't find it anywhere. THanks.

I had a patient who initially was unconscious and vomiting. He came to a bit and was able to say that he smoked marijuana and took "spices". Anyone had experience with that? Normal BP, tachycardic at around 145. Thanks
