In earnest, I wish more medics would become PAs instead of RNs (unless they are pursuing NP positions). The prescriptive authority exits as does a level of prestige and respect with physicians that might not exist with RNs (sadly).
So the individual EMTs nor their officers, commanders, physicians, etc can be sued, you can sue the squad as a corporation/entity. The solution is simple, in that case. Simple close the squad and reopen the next day as Plainfield EMS Squad or some such, with the same address, same people, etc...
I haven't. I think there is a pretty strong ethical argument to be made that it isn't allowed if you're primary interaction with this person is as your patient. If you already know them previous to your EMS interaction with them then it might be ok, but you should still seek an ethical consult.
