    ECMO truck

I think there are two different things being discussed. There are programs that are doing true pre-hospital ECLS cannulations and then there is what Tigger is describing as what some call "ECMO Retrieval" in which the ambulance is just a means to get the ECMO team and equipment to an non-ECMO...

    ECMO truck

I am a huge advocate for ECMO however field ECMO is just a flex and waste of resources. Have a robust ED ECMO program and encourage early transport with mechanical CPR for appropriate patients Most intra-arrest ECPR are shotgun fem-fem VA. I am assuming they will have pre-primed circuits...

    Medic Ambulance ALS-RN

I have no first hand experience however with talking to friends in CA it seems that like stated above many of the "CCT-RN" transfers are typical ALS stuff and not what most would consider actual critical care.

Correct, at appropriate dosages the duration of action is pretty similar with Vec being slightly longer and the onset being the bigger difference. I guess a more general statement would be that a short acting sedative with a long acting paralytic isn't an ideal combination for inexperienced...

Very high likelihood the patient will be paralyzed and not sedated either during the peri-intubation period or thereafter. Sure you can time it appropriately and quickly follow up with adequate sedation however in our area it is always Etom/vec and 30-45 minutes later think about adding sedation.
