    COVID VACCINE - The Megathread

OK, it’s months late but Ive been super busy, distracted and not paying attention to some threads. Both my wife and I took the Pfizer (free). After the first one, I wanted to rip my arm off and beat the person who have the shot to me with it. Second, no problem. Mild general ache but meh...

    Refusal on possible SI patient?

If the caller wants to file a petition to have law enforcement assist with an involuntary mental health evaluation (Pennsylvania 302), we’ll wait. I can not lawfully state “in the name of the department of health, I hereby medically detain and kidnap you.”

Did I cause barotrauma to a pt?

Contrary to TV, most codes arent successful. CPR breaks bones and contuses stuff inside. Bagging patients while CPR is being done does cause damage to the lungs.. But, not doing that and dead is dead and not just "mostly dead." Decades ago, we had a frequent flier. Woman who would brady...

Can you genuinely get Manual Vitals in every scenario?

There are a few times when you cannot get vital signs. Basically, when they don't have them Say: cardiac arrest, chainsaw massacre or all limbs blown off, they're glowing and the hazmat team is still trying to decon, before they've been brought of of the snake pit at the zoo, while you're...

How many paramedics dump ice on hyperthemic patients in the field?

a few years ago, I saw an ad on Ems world.. 3 year base contract. I’ve been here 5.25 years so far. Personal life, it’s insanely amazing and I now have fraternal twins to a complete Turkish babe. The system is amazing when it works, and is a dozen nationalities servicing three dozen...

How many paramedics dump ice on hyperthemic patients in the field?

So, the nutshell version, if I understand... If you're dealing with a fairly young, healthy population that just got jacked on bullets, blown up, or crimped in a car, it's probably very therapeutic in the initial management especially when we can't run low titer whole blood. The older they...

    Video Laryngoscope Thread

We still carry traditional steel, and the McGrath. Glidescopes are usually available, I'll usually grab one if I can. I've used the McG a bunch of times, and it is my go to unless I see utter airway hell and I know I'm gonna be tossing a salad and using a shop vac. It's more "normal" than the...

Joining the USAR as a 68w; Worth it?

The Reserves don't quite train John Wick to also do medical stuff... Active (possibly) but plan on either 18X(D) or 68W/18D.. or Guard with 19th or 20th Group.

    Praying while on call

Yeah, that's patient driven. Although once or twice I may have done something along the lines of "Hail Mary, full of Grace, let this (IV/ETT) slide into place." Another time or two, there could have been a quick prayer caused by the partner's driving or the helo's flying.

    Still alive?

Unless there are major indications of permanent death, I'mma gonna get that 4ld ECG strip just because I can. Traditionally did the 30 second listen for any cardiac tones. Wouldn't mind a POCUS on the heart, but hey. Excluding: Decapitation, incineration, decomposition, rigor mortis...

We drop two amps (total 1 gram) in a 100 NSS and 5-10 minutes. Generally it's set to a quick drip and just ran in the ballpark.

What has been your experience with language barriers?

I'm in a very crazy system. Qatar. There are an insane number of languages living here, particularly in the industrial/ laborer neighborhoods. The "majority" can speak arabic and/or english, but... the lower level workers? Nope. Good luck with that. That's what they have supervisors who can...
