    Media v. Cops and Docs

I have not connection to Minnesota EMS so will withhold any commentary on this "story." What I will say is that EMS is not immune to pressure from law enforcement to utilize Ketamine in situations where pharmacological restraint is not appropriate. I am not saying that this happened in...

    Personality assessment

My department does a pysch interview for each candidate that gets an offer. Included in this is the California Personality Index. To my knowledge, the department does nothing with the results of this test but they do get the report. The psych profile to me mostly felt like a polygraph, the...

What do you carry? - The mega thread

Small Gerber locker, my phone, and the ride phone. I used to carry a pharmacopeia and some cheat cards but with the four pocket pants that isn't going to happen. I have my Mountainsmith lumbar pack on the ambulance with all the stuff I used to carry in my pants and snacks. 90 degree flashlight...

IFT Post stroke pt - does ems need to get tpa administration time?

You continue to provide excuses as to why it wasn’t important for you to have this information. It is. Though the “why” has been provided, sometimes it’s also worth remembering that the why doesn’t really matter and you are still responsible, like it or not.

freezing up and choking during skill drills

I was responding to another post regarding the fact that trauma surgeons do not do simulations, which is patently untrue. Also if one’s anxiety prevents performance in front of a group of people, couldn’t that also hold true for I dunno, FTOs, other responders, and families? Are these folks not...

Why Burn first aid is as important as any advanced life support treatment and why US EMS stinks at burn first aid.

Just because something is not in EMS guidelines does not mean it isn’t best practice. Guidelines take a long time to change compared to consensus.

IFT Post stroke pt - does ems need to get tpa administration time?

Dude get over yourself. You need to be in possession of some kind of list of meds that the facility gave. There’s a litany of reasons, perhaps some matter and some don’t, but who cares. Learn and move on. Most IFTs are total non events. Until you end up in the ED of some unfamiliar hospital (or...

    Resp failure ift pt

Oof. That’s awful. Perhaps these are not the most comparable cases, but the more you ride the edge, the more likely you are to take a bigger chance.

IFT Post stroke pt - does ems need to get tpa administration time?

If you are doing anything close to emergent (and really all) IFTs, you should have a copy of all the medications the facility gave with the time and dose given. A report is of course called, but that doesn't guarantee you'll see the same care team at the receiving. Even in network, it's not...

Why Burn first aid is as important as any advanced life support treatment and why US EMS stinks at burn first aid.

I think of all the things he is trying to do, selling things is not it. Pretty much all the evidence I am familiar with specifically emphasizes the futility of using fancy bandages or salves. Essentially a non-stick and bacitracin is the consensus of most burn forums.

    Resp failure ift pt

A word to the wise, don't seek feedback for calls and then say "we did the best we could." Maybe, maybe not. Perhaps it would have been better for the patient to remain at the facility on BiPap until a CCT team could have transferred the patient. Don't take patients that are not appropriate just...

Why Burn first aid is as important as any advanced life support treatment and why US EMS stinks at burn first aid.

There's nothing I like more than being told I am offended, when I am most certainly not. Also I don't think anyone here is really telling you that you're wrong. So there's that. Would it make you feel better if I told you that there's lots of things that I don't like about my guidelines? That...

Why Burn first aid is as important as any advanced life support treatment and why US EMS stinks at burn first aid.

Frankly your delivery sucks. This is not how you go about talking about EBM, when you lead off with "I'm right, you're wrong," you don't exactly leave much room for discussion. Sure you didn't attack any individual, you just said we all suck. Which you're probably right about, but that's not...

    the 100% directionless thread

Must be nice to get tubes... I think I like the regular three more is mostly just that it displaces more tongue with less movement. To me, most of the people who need a hyperanglulated blade also have enourmous tongues, and the narrow profile of that blade is just a slight bit more of a...

freezing up and choking during skill drills

I wasn't aware that the whole medical model of education included taking folks straight from medical school and into the trauma bay. Must be a wild first day. Even if that was happening, it wouldn't make sense for EMS with its regrettably low contact hours. Maybe see one, do one, teach one is...

    Scheduling software?

I am but a user. However, CrewSense (VectorSolutions) is certainly the best I've used so far. Their app is excellent and in terms of "just" scheduling, I can't think of anything it lacks. Another job recently got EPro which is fine, but the mobile interface is garbage. The time keeping portion...

freezing up and choking during skill drills

You don't. There is no reason to even try to simulate a situation that will rarely, if ever happen to most providers. A good program tries to prepare its students for their primary responsibilities. A certain amount of both critical thinking and experiential learning is still needed to take what...

freezing up and choking during skill drills

But the folks that cannot perform skills in a classroom environment can rarely just make it happen on the street. If you "choke" on the skill in the classroom, you are missing the opportunity to learn to do something with a bit of added stress, the sort of stress that doesn't really approach...
