negative pull-ups are a great way. Also don't go to exhaustion. I prefer to pick a number I can hit repeatedly (15) and do three sets of them. Only "max" once and a while

Depending on the amount of difficulty breathing, my response would be different. If it's only a little difficulty, keep head in position you found it and pad it as best you can, just like treating an elderly patient with a deformed spine. I'd pad, put pt. on O2 via NRB, or if need be BVM him...
